on Enrollment for full information.Registration ProcessEvery student must have a completed Student Registration form to attend school. A copy of the Student Registration form may be found here. To complete the form you may type in the requested information and then print the completed form, or you may print out a blank form and write in the requested information.Please bring a completed and signed Student Registration form to your neighborhood school along with: 1. Evidence of immunization for diphtheria, tetanus, polio, hepatitis B, varicella or chickenpox disease, Hib, mumps, rubella, pertussis, hepatitis A and measles. These are required unless parents wish to seek an exemption because of medical or religious reasons. Vaccine requirements vary by a child's age and grade level. Please reference 'Vaccine Requirements for School Year 2008-09' for specific grade level requirements" and your child's school before enrolling by visiting http://www.mesd.k12.or.us/shs/main/parentinfo/index.html 2. Proof of age for the student. A birth certificate or other similar document is sufficient. 3. Proof of home address. Two or more documents such as utility bills, rental agreement, or other bills to the home address. These must be recent and the name must be of a parent or guardian of the student. 4. Contact information for a previous non-Portland Public School attended (if applicable). Students should be registered at their neighborhood school as early as possible. Most schools will hold informational meetings to provide information and guided tours of the school building, please contact your neighborhood school for more information.,br. ESL students (students who are not native English speakers and have some difficulty with the English language) must go through an evaluation. Middle School StudentsFifth-grade students attending a Portland Public School will be automatically enrolled in their neighborhood middle school the following school year.
